Welcome to Polk County's Community Services Division! We have been undergoing restructuring the past few year in order to improve the quality of health and human service delivery to the citizens of Polk County, Wisconsin. Our services range from health promotion and disease prevention to assessment, treatment and recovery services for those experiencing substance abuse and mental health conditions, to family support and crisis intervention services for children and adults in need of protection and safety. Our staff are committed to serving Polk County citizens with respectful client centered care focused on empowering individuals and families to be healthy and self-supporting.
